A hallucination is when an AI model states something false or made-up as if it were true, such as a fake statistic, quote or citation. It happens because models generate plausible text, not verified facts. Reduce it by giving sources, allowing 'I don't know', and checking claims before use.
IBM describes AI hallucinations as outputs that sound plausible but are factually wrong, irrelevant or entirely fabricated. Examples include invented studies, URLs that don’t exist and wrong details about real people or companies. It’s a known property of language models, not a rare bug.
